An organizational maturity model is a framework that describes how developed a company's capabilities are on a fixed ladder, usually five levels running from Initial (ad hoc and reactive) to Optimized (measured and continuously improving), so leaders can see where they stand and what the next stage looks like. The classic five levels, drawn from the Capability Maturity Model, are: Level 1 Initial, where work is unpredictable and depends on individuals; Level 2 Managed or Repeatable, where basic processes exist and get followed on known work; Level 3 Defined, where processes are documented and standardized across the organization; Level 4 Quantitatively Managed, where you measure performance with data and control it; and Level 5 Optimizing, where the organization improves itself deliberately and continuously. The value of a maturity model is that it turns a vague worry ("we feel disorganized") into a position on a ladder with a defined next step. Its limit is that most models are a description, not a measurement: they tell you what Level 3 looks like, but not whether you are at Level 3, and never whether Level 3 is normal for your industry. A maturity model is the framework: the ladder of levels and what each one looks like. A maturity assessment is the act of measuring where you actually sit on that ladder. The model is a map; the assessment is finding your position on it. Many teams have the model and skip the assessment, which is why they can describe Level 4 but cannot say whether they are at Level 2 or Level 3 today.

CMMI (Capability Maturity Model Integration) is the best known process maturity model, and its five-level structure is where most other maturity models get their ladder. But a formal CMMI rating requires a certified appraisal by a licensed lead appraiser, which is a different exercise from a diagnostic self-assessment.
