The construct comes from Amy Edmondson's research at Harvard Business School, first published in Administrative Science Quarterly in 1999, and Google's Project Aristotle later found it was the strongest single predictor of team performance in its own data. Most instruments on the market measure it inside one intact team: Edmondson's own licensed Fearless Organization Scan, LeaderFactor's four-stage model, and various team self-assessments all work that way, and for a single team that is the right tool. Questions about psychological safety assessment

Psychological safety is a shared belief that the team or organization is safe for interpersonal risk taking: that you can admit a mistake, ask a naive question, disagree with your boss, or flag a problem without being humiliated or penalized. Amy Edmondson defined it in her 1999 research. It is not about comfort or being nice, and it does not mean lower standards. High-performing groups combine high safety with high accountability.

You measure psychological safety with an anonymous survey that asks people to rate statements about speaking up, admitting error, asking for help, and raising bad news, then you score the responses and compare them to a benchmark. Edmondson's original scale uses seven items with three reverse-scored. The critical design points are anonymity, consistent wording over time, and a benchmark, because a raw average means nothing without something to compare it to.

The strongest questions describe a specific behavior rather than a feeling. Ask whether people can raise problems and tough issues, whether mistakes are held against them, whether it is safe to take a risk, whether it is difficult to ask others for help, and whether unique skills are valued. Judge it against a benchmark rather than an absolute.
